ASSESSMENTS

Sage Prairie offers scheduled and walk-in assessments at both of its locations (from 9am       to 2pm). A Comprehensive Assessment is a clinical tool used to determine substance use disorder need based on the American Society of Addiction Medicine’s guidelines. This tool can be used for placement in a facility, determine level of care, “rule in or rule out” substance use disorder, admit to treatment, or to see if a person would benefit from substance use disorder treatment care.

COUNSELING

“Relapse” does not need to be a part of recovery. Sage Prairie uses evidence based counseling models, including cognitive behavioral care, motivational interviewing, mental health groups, and professionally led peer groups. Using these techniques, and continually assessing length of treatment stay, most patients are engaged in some level of service (either at Sage Prairie or referred from an outside entity) for 8-10 months; patients step down in intensity gradually based on individual patient needs, while respecting individual autonomy.

MENTAL HEALTH

We offer both medication management for mental health and psychotherapy. We all know there is not a “one size fits all” therapist. This is why we partner with local providers to ensure each patient is receiving the care they need based on their psychological needs. This also ensures that once a relationship has been established, the individual can continue to see their provider after treatment; it also ensures each person can “switch” therapists or continue to see their own provider.

MEDICATION ASSISTED TREATMENT

Most individuals diagnosed with a substance use disorder increase their chances of success in recovery when on medications to support recovery. This does not mean that medication management is right for everyone. We have partnered with providers who specialize in Mental Health and Addiction to see where medication may increase individual success. Some of the evidence based medications offered include suboxone, Naltrexone (including Vivitrol), and other evidence based medication to decrease effects of withdrawal or decrease cravings. All patients are seen by their own provider or one of our trusted partners.

PEER SUPPORT

A Certified Peer Recovery Specialist (CPRS) is an individual whose role it is to provide support and reassurance in recovery. They are non-clinical. These individuals are established in their own recovery to help, alongside, the individual to find resources, offer an empathetic ear, and help reduce harm from disordered use. Sage Prairie has incorporated this care into all levels of programming. If needed, a patient can be referred to local Recovery Community Organizations to meet needs not offered by the CPRS’s employed by sage prairie (culturally specific needs, alternative approaches, etc.).

ONGOING CARE

Recovery does not end with detox. To be effective, treatment length is best to be assessed throughout the treatment stay. Once a patient is stabilized, their length of stay is addressed routinely. The length of stay is based on patient needs (housing, medications, meeting goals, family needs, collaboration with other professionals, etc.). Studies routinely find that the longer an individual remains engaged in treatment, are more likely to achieve maximum lifetime benefits. Recovery does not end when treatment ends. Sage Prairie offers ongoing peer support groups at each location through partnerships with the local Recovery Community Organization, South Metro Community Services
